Machine "killed Royal Hospital patient"

The failure of the equipment at the Royal is now being investigated by Liverpool coroner Andre Rebello and theŠMedicines and Healthcare products Regulatory Agency (MHRA).

The device has been sent to the manufacturer for microscopic inspection.

The hospital trust's legal department is also dealing with the death of another patient who fell from a window within its premises.

The coroner is also investigating this case.

Trust documents read: "Investigations have commenced into the death of a patient following the failure of a device during a procedure.

"The MHRA and Coroner are investigating the matter.

"The device has undergone a preliminary inspection and has now been sent to the manufacturer for inspection under microscopic conditions. "A mention was held on September 16 at which the family were represented by a barrister and a solicitor.”

The document continued: "A further mention was held on September 30 into the death of a patient who died following a fall from a window.

"The police are to provide theŠCoroner with their report by November 17.

"The Coroner will then review the report to identify any further issues which may need to be addressed.

"We are advised this will take approximately four weeks.”

A spokeswoman for the Royal said it was unable to comment until the coroner's investigations have concluded.
